Job Summary

  • Provide leadership, supervision, and support to a team of assigned staff providing services to consumers enrolled in Tennessee Health Link.
  • Ensure consumers are seen upon admission and discharge from hospitals, diversion services, or residential treatment facilities, and that their daily needs are planned for and coordinated.

Key Requirements

  • Master’s degree in health-related field preferred
  • Bachelor’s degree in health-related field
  • Minimum 6 months care management team leader experience
  • 1+ years care management tier experience
  • Valid Tennessee Driver’s License
  • Acceptable Motor Vehicle Report
  • Acceptable Criminal Background Investigation
