Job Summary
The Medical Social Worker provides patient/family psychosocial consultation, interventions and resources which may include, but not limited to, Social Drivers of Health (SDOH), family and community support systems, substance use, and protective factors.
The Social Worker works closely with the RN Case Manager/Transition Planner and interdisciplinary team to ensure safe and timely discharge.
Completes psychosocial assessments, identifying patient needs related to suicidality, social determinants of health, protective factors, substance use, mental health, family dynamics and barriers and goals related to safe discharge.
