Key Requirements

  • Bachelor’s Degree in Nursing or Social Work
  • Registered Nurse (RN) license preferred
  • Licensed LVN or LPN nurse acceptable
  • Two years clinical nursing experience preferred
  • Knowledge of Medicare, Medicaid programs
  • Valid LVN/LPN or RN license in state of work
  • Certificate as certified Case Manager (CCM) a plus
  • Maintain continuing education and licensing
