Job Summary
The Case Management Extender PFK helps navigate and access community services and other resources and provides support through maintaining population health programs and care coordination activities.
Case Management Extenders are patient and family facing with daily activities during inpatient stays, outpatient specialty clinic visits, emergency room visits, home, community visits, and via telephone.
Connects families to resources to address social determinants of health and accommodates the specific cultural and linguistic needs of all patients.
