Job Summary
The role is responsible for providing high quality, overall physiotherapy management to patients admitted within the Transition Care Program.
Applications from current grade two physiotherapists or suitably experienced grade one physiotherapists will be considered.
The Transition Care Program provides short-term support and active management for older people at the interface of the acute/subacute and residential aged care sectors.
