Job Summary

  • The Community Health Visitor will deliver health and social integrated care for mother and child, focusing on antenatal and postnatal care, developmental-centred parenting, parent-child interaction and family well-being.
  • The CHV is the keyworker who will support the mother-child dyad and family through home visits and clinic service touchpoints, also providing service coordination between patient/client/family with healthcare and social service systems.
  • To collaborate and partner community and social service agencies, or hospital specialists to provide necessary support for client and family.
