The role involves tracking and monitoring facility effective utilization of PDPM tools including Physician Diagnosis Verification and admission/discharge reports.
The incumbent must assess and determine the health status and level of care of all new admissions while ensuring regulatory compliance.

Skills & Requirements
Must-have
Oversee weekly PDPM meetings for skilled patients
Ensure accurate and timely MDS assessment completion
Coordinate interdisciplinary participation in resident assessments
Maintain confidentiality of protected health information
Track QM and QASP performance monthly and quarterly
Nice-to-have
Experience with PCC software preferred
Long term care experience preferred
Ability to provide training to new coordinators
Strong data analysis skills for clinical recommendations
