Job Summary

• Pair Team is a tech-enabled medical group reimagining how Medicaid and Medicare serve underserved populations by delivering whole-person care, leveraging AI and automation to reduce administrative burden.
• The Lead Care Manager will maintain an ongoing caseload, engage members with Serious Mental Illness/Substance Use Disorder, homelessness, or high medical needs, and support them in achieving health goals.
• This role primarily allows for remote work, with 2-3 in-person visits per month in the community, requiring reliable transportation and a quiet, HIPAA-compliant internet-connected space.
