Key Requirements
Bachelor’s degree in public health or related field with 10 years experience
Licensed social worker with 8 years experience
Surveyor Minimum Qualifications Test (SMQT) certification preferred
Completion of Federal/State Nursing Facilities training including SMQT within one year
Ability to travel extensively including overnight stays
