Job Summary
The Case Manager II provides intensive, client-centered case management to Guests experiencing complex and high-acuity needs, focusing on elevated support, coordination of care, and system navigation.
Key responsibilities include engaging Guests with complex vulnerabilities, coordinating services across internal and external systems, and supporting Guests in achieving personal goals like housing stabilization and health access.
The role requires a Degree in Social Work, registration with NLASW, and a minimum of 2 years' experience in case management, preferably with marginalized or high-acuity populations.
