Job Summary

• The School Social Worker leverages clinical expertise to conduct comprehensive psychosocial assessments, create and implement individualized treatment plans, and contribute to the development and implementation of Individualized Education Plans (IEPs), delivering exceptional care to day treatment patients.
• This role collaborates closely with patients, families, healthcare providers, insurers, community resources, and other stakeholders to assess patient needs, deliver services, coordinate care, develop transition plans, and advocate for patients.
