Job Summary
The RN Care Manager provides longitudinal care management services, by phone and face to face, to promote wellness, improved outcomes and self-care within a defined population of patients with chronic condition(s), complex health needs, and support their appropriate utilization of health services.

Key Requirements
3 years RN experience
Current BLS certification
Valid Arkansas Registered Nursing License
Baccalaureate Degree, ADN, or Diploma RN Degree
