Job Summary

  • The primary purpose of this position is to oversee the facility to ensure assessments are completed per guidelines focusing on the resident assessment process and plan of care.
  • The role includes providing oversight on weekly PDPM meetings, tracking performance metrics, and coordinating interdisciplinary participation to ensure thorough documentation and regulatory compliance.
  • This position requires maintaining confidentiality of resident information, conducting resident care conferences, and assisting in formulating and revising individualized care plans.

Key Requirements

  • Good standing with State Board of Nursing
  • Maintain continuing education/licensing
  • 3 years supervisory experience preferred
  • Long Term Care experience preferred
