Job Summary
As a Clinical Care Partner your role is to ensure that our clients receive person-centred, high-quality care by overseeing and coordinating complex clinical and care needs through assessment, care planning and coordination of services.
You’ll spend the majority of your day on the road, engaging with clients within the community.
