Job Summary

  • This job assists providers with the medication refill and infusion processes by ensuring patient’s compliance with medication, lab monitoring, adherent to therapy by providing appropriate refill amounts on medication refill requests, maintaining accurate pharmacy preferences, identify discrepancies or problems with medication history information.
  • The role communicates with patients, families, caregivers, healthcare providers/practitioners, and pharmacists to obtain and accurate and complete medication history for patients while also documenting the encounters within the electronic health record (EHR).
  • Accurately obtains and clarifies patients' home/chronic medication list by contacting community pharmacies, physician offices and/or extended care facilities, interviewing patient and/or family members to ensure proper identification of doses and drugs including prescription medications, over the counter drugs and alternative medications in a timely manner.

Key Requirements

  • High School diploma or equivalent
  • 2 years of experience as a Pharmacy Technician or equivalent health care experience
  • 1 year of college or completion of a para-professional pharmacy training program (Preferred)
  • Prior experience taking patient medication histories (Preferred)
  • Certified Pharmacy Technician (Preferred)
