Job Summary

  • In collaboration with the attending physician, the Geriatric Nurse Practitioner will provide care to patients in one or more clinical venues of the Hebrew Rehabilitation Center.
  • Performs initial and periodic assessments of patients and appropriately identifies and documents problems and issues in the medical record.
  • Participates in quality assessment activities, including but not limited to record reviews, outcome measurement, and review and revision of policies and procedures.

Key Requirements

  • Master's Degree required
  • Graduate of accredited Nurse Practitioner program
  • Licensed as a nurse practitioner by the Commonwealth of Massachusetts
  • National Certification as Geriatric, Adult, or Family Nurse Practitioner
  • Four years of Nurse Practitioner experience preferred
  • Two years in a geriatric facility preferred
