  • Work collaboratively with clients and carers to support and empower them to remain as independent as they can and remain socially active within their communities.
  • Deliver services in the Community Rehabilitation Centre, the community setting and the client's home.
  • HIP aims to delivers improved outcomes for clients and to support hospital demand management and flow, through delivering integrated and coordinated care to clients across the hospital and community interface.

Social Worker Grade 2 SC21 - SC24 ($90,235 - $103,724 per annum pro rata)
