Job Summary

  • The primary purpose of this position is to oversee the facility to ensure all resident assessments are completed per guidelines.
  • The role requires tracking and monitoring facility effective utilization of PDPM tools including Physician Diagnosis Verification and Weekly CSR.
  • Candidates must ensure accurate and timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, and significant change evaluations.

Key Requirements

  • Bachelor of Science Degree in Nursing preferred
  • 3 years of supervisory experience preferred
  • Good standing with State Board of Nursing
  • Long Term Care Experience Preferred
