Job Summary

  • The primary purpose of this position is to oversee the facility to ensure all resident assessments are completed per guidelines.
  • The role requires tracking and monitoring facility utilization of PDPM tools while ensuring accurate and timely completion of all MDS assessments.
  • Candidates must be proficient in Microsoft Suite and have knowledge of PCC software to perform data entry and electronic submission functions.

Skills & Requirements

Must-have

  • Complete MDS assessments per regulatory timeframes
  • Monitor PDPM tools and QM performance

  • Assess new admissions health status and level of care

Nice-to-have

  • Experience with PCC software systems
  • Proficiency in Microsoft Suite products
  • Long term care facility experience preferred
  • Ability to provide clinical recommendations for outcomes

Key Requirements

  • Bachelor of Science Degree in Nursing preferred
  • Must be in good standing with State Board of Nursing
  • 3 years of supervisory experience preferred
  • Long Term Care Experience Preferred
