This role is responsible for assisting in the planning, developing, and directing of social service programs to meet the medically related emotional and social needs of residents.
The position requires coordinating social service activities with other departments and community agencies to ensure the highest practicable level of well-being for each resident.
Employees must maintain strict confidentiality of all resident care information including protected health information while providing essential social services.

Key Requirements
Bachelor's Degree in Social Work (BSW)
One year supervised social work experience in healthcare setting
State social work licensure (LSW, LMSW, LCSW) if required
Completion of mandatory in-service trainings including HIPAA
Knowledge of Medicare/Medicaid and financial assistance programs
