Job Summary

  • The primary purpose of this position is to oversee the facility to ensure all resident assessments are completed per guidelines.
  • This role involves tracking and monitoring facility utilization of PDPM tools and providing clinical recommendations for optimum outcomes.
  • The incumbent must assess new admissions, determine level of care, and coordinate interdisciplinary participation in completing the Minimum Data Set.

Skills & Requirements

Must-have

  • Complete MDS assessments per regulatory timeframes
  • Monitor PDPM tools and QM performance
  • Coordinate interdisciplinary care conferences
  • Ensure accurate data entry and electronic submission
  • Maintain confidentiality of protected health information

Nice-to-have

  • Experience with PCC software systems
  • Proficiency in Microsoft Suite products
  • Long-term care experience preferred
  • Ability to provide training to new coordinators

Key Requirements

  • Bachelor of Science Degree in Nursing preferred
  • Graduation from an Accredited School of Nursing
  • Good standing with State Board of Nursing
  • 3 years of supervisory experience preferred
  • Long Term Care Experience Preferred
