The Per Diem RN Care Manager will participate in a dynamic interdisciplinary team to provide case management services to identified complex, high-risk patients across the care continuum.

Key Requirements
Bachelor’s Degree required
Valid OR RN License required
2 years related Care Manager experience or 5 years nursing experience
