Job Summary

  • Provides clinical social work services to patients and their families enrolled in Hospice, Home-Based Palliative Care program and to select patients and their families receiving services from Homecare.
  • Responsible for the Hospice Volunteer and Hospice Bereavement programs, and provides consultation and education to staff regarding psychosocial issues.
  • Collaborates with the interdisciplinary team and the family in the development of appropriate family centered case plans following the Medicare Condition of Participation Guidelines.
