Job Summary
Provides clinical social work services to patients and their families enrolled in Hospice, Home-Based Palliative Care program and to select patients and their families receiving services from Homecare.
Responsible for the Hospice Volunteer and Hospice Bereavement programs, and provides consultation and education to staff regarding psychosocial issues.
Collaborates with the interdisciplinary team and the family in the development of appropriate family centered case plans following the Medicare Condition of Participation Guidelines.
