Job Summary
Plans, monitors, and coordinates individual patient care plan utilizing critical pathways and best practice guidelines for both inpatient and outpatient needs.
Acts as the primary point of contact for patients and their families assisting with all capacity of care, triage concerns, medication education, medication refills, and all other requests within scope.
