The Case Manager provides intensive home-based intervention case management services using a Trauma-Informed Care approach to assist youth in the Transitional Housing Program.
Employees are responsible for developing, monitoring, and reviewing Individual Treatment Plans while responding promptly to crises situations with clients at the office, during home visits, or on phone calls.
The role requires maintaining confidentiality, complying with federal and state regulatory guidelines, and working flexible hours including evenings, weekends, and holidays when necessary.

Key Requirements

1 year of intensive in-house-based interventions experience
LMFT, LPC, LCSW, LMFT-A, or LPC-I license
First Aid and CPR certification
Pass pre-employment drug screen and criminal background check
Proof of work eligibility status required
