Job Summary

• The ECM Outreach Coordinator is responsible for community outreach and enrolling eligible clients into Enhanced Care Management (ECM), a Medi-Cal managed care benefit that addresses the clinical and non-clinical needs of high-need individuals.
• This position works collaboratively with multidisciplinary team and community resources.

• High School Diploma or GED required
• One year or more of case management or related experience required
