Job Summary
The Service Coordinator II supports maximizing the Member’s health, wellbeing, and independence by focusing on person-centered care planning and assessing service and support needs.
This role facilitates clinically appropriate and fiscally responsible patient care through communication with physicians, family, caregivers, and other healthcare team members.
The Service Coordinator II is responsible for assessing Member behavioral health needs, coordinating services, and completing various evidence-based assessment tools to meet State contractual requirements for STAR Kids.
