Must-have
Bachelor's degree in Social Work or Nursing
Experience in healthcare case management
Knowledge of Medicare Medi-Cal Medicaid programs
Strong communication with residents and families
Ability to manage multiple tasks in fast-paced environment
Nice-to-have
Familiarity with PointClickCare EMR systems
Certified Case Manager (CCM) credential
Compassionate resident-centered care approach
Intermediate computer skills for documentation
Key Requirements
Bachelor's Degree in Social Work, Human Services, Nursing, or related field
Prior experience in healthcare, case management, or social services
Knowledge of Medicare, Medi-Cal, and Medicaid benefits
Non-licensed candidates accepted if qualified
Certificate as Certified Case Manager (CCM) is a plus
